Glycopeptide Anti-bacterialsIndication Dosage
Serious Infections Caused By Gram-positive Bacteria Such As Complicated Skin And Soft-tissue Infections, Pneumonia:
By IV Inj, By IV Infusion, or By IM Inj:
- Adult (body weight Up To 70 Kg): Initially 400 Mg Every12 Hours For 3 Doses, Followed By 400 Mg Once Daily; (body weight 70 Kg And Above): Initially 6 Mg/kg every 12 Hours For 3 Doses, Then 6 Mg/kg Once Daily
- Children: Neonates: 16 Mg/kg Body Weight As A Single Dose By IV Infusion Over 30 Min On First Day, Then 8 Mg/kg Body Weight As Single Daily Dose; Over 2 Months: Moderate Infections: 10 Mg/kg Body Weight By IV Injection Every 12 Hr For First Three Doses, Then 6 Mg/kg By Iv Or IM Injection As A Single Dose Daily; Severe Infections: 10 Mg/kg By Iv Or IM Injection As A Single Dose Daily.
Streptococcal Or Enterococcal Endocarditis (in combination With Another Antibacterial):
By IV Inj, Or By IV Infusion:
- Adult: Initially 10 Mg/kg Every 12 Hours For 3-5 Doses, Then 10 Mg/kg Once Daily, Subsequent Doses Can Be Given By Intramuscular Injection
Bone And Joint Infections:
By IV Inj, Or By IV Infusion:
- Adult: Initially 12 Mg/kg Every 12 Hours For 3-5 Doses, Then 12 Mg/kg Once Daily Subsequent Doses Can Begiven By Intramuscular Injection, Increased Risk Of Fever and Rash With Doses Of 12 Mg/kg
Surgical Prophylaxis:
By IV Injection:
- Adult: 400 Mg, To Be Administered Up To 30 Minutes Before The Procedure
Surgical Prophylaxis In Open Fractures:
By IV Infusion:
- Adult: 800 Mg, To Be Administered Up To 30 Minutes Before Skeletal Stabilisation And Definitive Soft-tissue closure
Peritonitis Associated With Peritoneal Dialysis:
By Intraperitoneal Administration:
